The Ansett-ANA Douglas DC-6B VH-INA suffered a major propellor failure on the Number 3 engine, as it was taking-off from Melbourne (Essendon) en-route to Adelaide (Parafield). The unbalanced engine was partially wrenched from its upper engine mounts, posing a major hazard for the pilots if a landing was attempted with the engine still attached to the airframe.
